Peterson Health’s Providers at Home has been chosen by the Centers for Medicare & Medicaid Services (CMS) to participate in a new Medicare alternative payment model aimed at supporting individuals living with dementia and their caregivers. The program, known as the Guiding an Improved Dementia Experience (GUIDE) Model, involves nearly 400 participants nationwide in developing Dementia Care Programs (DCPs). The initiative focuses on enhancing care coordination and expanding access to services such as respite care for dementia patients and their caregivers.
CMS Administrator Chiquita Brooks-LaSure expressed enthusiasm for the partnership, highlighting that the GUIDE Model represents a new approach to Medicare payments for dementia care. The model aims to support both individuals with dementia and their caregivers, reducing the strain on caregivers and enabling more people to remain in their homes and communities rather than moving to institutional settings.
The GUIDE Model, which began on July 1, 2024, will test a new payment structure for essential supportive services including comprehensive, person-centered assessments, care coordination, and 24/7 access to care team members or help lines. The program will also provide respite services for caregivers. Additionally, Care Navigators will assist individuals and caregivers in accessing clinical and non-clinical services such as meals and transportation through community-based organizations.

Participants in the GUIDE Model include a diverse range of healthcare providers, such as academic medical centers, small group practices, community-based organizations, health systems, and hospice agencies.
